Covid-19 restrictions cause CervicalCheck samples to go out of date

Two hundred women will have to be retested due to delays in processing tests

Two hundred women will have to be retested by CervicalCheck after their samples went out of date due to issues caused by Covid-19 social distancing restrictions.

Clinical director Dr Nóirín Russell said the restrictions led to the delays in the sample processing centre which in turn led to the expiry of the samples.

Speaking on RTÉ radio's Morning Ireland, Dr Russell said that the laboratory had to adhere to social distancing measures because of Covid-19, which meant fewer staff working and carrying out tests.

This delay led to the samples of 200 women going out of date, she added, so the women will have to be retested.


Dr Russell said that Human papillomavirus (HPV) was found in the women’s samples, but they were not sent on for cytology examination within the timeline period of 42 days.

The women will be contacted this week by letter, along with their GPs, to arrange retesting.

Dr Russell said it was very important that the women attend for the second test, but she emphasised that the presence of HPV in their sample did not mean they had cervical cancer.

It had been anticipated that there would be “huge” numbers coming forward for tests in 2021 after the service had to halt for a period in 2020 due to Covid-19, she said.

“We had hoped the impact from Covid would be less,” she said.

“Huge capacity issues” had led to some samples going past the 42 day window for cytology examination. Most of the tests had taken place in April and May, with a few in March, added Dr Russell.

“We knew this was coming,” she said, which was why measures had been put in place to ensure supports were in place, including a helpline which can be contacted at 1800 45 45 55.

Every month data from the laboratory was examined and it had been noticed that every month some samples were expiring, she said.

This increase in expired samples led to a site visit and it was determined that it would be necessary to prepare the women so they would not panic when they received the letter.

The process will prioritise the women in question, who will obviously be worried, she said.

Addressing the capacity issue would not be a “quick fix” as the social distancing guidelines still had to be observed. “We are trying to work very closely with the sample takers to steady the flow of samples so there are no peaks and troughs.”

CervicalCheck chief executive Fiona Murphy said the service will not hide any details from the public and it was holding their hands up and offering an apology to the women who will have to do a repeat smear test because their samples expired.

“Samples need to make their way through the system in a timely way. We can only say sorry that we didn’t for these 200 women,” she told RTÉ radio’s Today with Claire Byrne show.

It was part of the trust and confidence agreement with the public that the service would reveal any problems that arose, she added.

As a rule, 13 per cent of smear tests needed to be sent for cytology testing, but to reach that, 100 per cent of smears had to be examined, explained Ms Murphy. It had been expected that 140,000 smear tests would be carried out earlier this year, but 200,000 were completed. Additional testing capacity had been organised, but they were still unable to get through the samples fast enough, she added.

The laboratories had added extra capacity and were completing 5,000 to 6,000 tests per week, but samples expired “all the time”, but normally this figure would be much lower, she said.

Samples needed to make their way through the system in a timely way, but that had not happened in this situation, she acknowledged. “We are looking at how to speed up the process.”

Advocacy groups had been informed last week of what had happened while GPs and the women would be informed this week. “We’re caught up now,”

Ms Murphy said that a three month delay was low risk as was the possibility of cervical cancer for the 200 women involved.
