THE CHILD in this case was less than a year old when he sustained serious brain injuries in a tragic incident involving his mother, but his family had been known to the HSE for two years before the childs’s death.

Records show there were serious concerns the child’s mother was at risk due to severe domestic violence.

She also had serious ongoing metal health problems and had been admitted to a psychiatric hospital a number of times in the months leading up to the incident in which the child suffered severe brain damage.

He was admitted to hospital where he remained until his death six months later.

Care of the remaining children was then taken over by the extended family.
