Two investigations are underway following the recovery of the body of a male psychiatric patient from the River Shannon after a three-day search.

He has been named as 40-year-old Mr John Gallagher from Cresslough, Letterkenny, Co Donegal.

Mr Gallagher went missing while being transferred back to Donegal from an appointment at a Dublin hospital. One of the two nurses escorting him was a student nurse.

North Western Health Board chairman Mr Sean McGowan described Mr Gallagher's death as tragic.
