19.15-19.30:Transplant co-ordinator for King's College Hospital, London, contacts the McGivern family about an available donor liver and tells them to get ready to travel.

19.30-19.45Co-ordinator asks Emergency Medical Response Services (EMSS) that Meadhbh be transferred as soon as possible.

19.45-20.00EMSS contacts the HSE National Ambulance Service requesting an air corps air ambulance.

They contact Meadhbh’s father and advise him to “stay put” and they contact UK air ambulance broker about availability of a private ambulance.

20.00-20.15HSE ambulance service contacts Air Corps requesting transport from Leitrim to London with possible pick-up from Knock. Air Corps says no aircraft is available.

Ambulance service contacts the Irish Coast Guard requesting a helicopter but the Coast Guard has none available until midnight.

Our Lady’s Children’s Hospital in Crumlin is advised that transport is not available.

20.15-20.30Emergency response service asks hospital transplant co-ordinator to assist in organising transport.

20.30-20.45Crumlin hospital contacts its air ambulance provider requesting ambulance.

20.45-21.00Crumlin hospital told that a jet has been sourced. Emergency response service requests RAF assistance.

21.00-21.15HSE ambulance service asks Coast Guard for use of Sligo-based helicopter.

RAF assistance not available .

21.15-21.30Meadhbh's father contacts emergency response service with contacts for private helicopter provider in Galway.

Call to provider is diverted.

21.30-21.45London hospital says it cannot source an aircraft.

Crumlin hospital’s air ambulance provider confirms it has sourced aircraft and crew with transfer from Knock to London but earliest departure time is 23.30.

21.45-22.00Coast Guard tells HSE it may have helicopter available from Sligo. HSE is told Coast Guard has secured jet but it is still in Europe.

Coast Guard confirms availability of Sligo helicopter and acceptance of the mission.

Crumlin hospital contacts its air ambulance provider and cancels jet.

22.00-22.15Emergency response services tells Meadhbh's family Coast Guard helicopter is available from Sligo and asks them to travel there immediately.

Coast Guard unable to give HSE an estimated time of arrival in London.

HSE tells Crumlin hospital provisionally aircraft would leave before 23.00 with 90 minute flying time to London.

HSE asks emergency response service to arrange road transfer from Heathrow to King’s College Hospital.

Crumlin hospital tells hospital transplant co-ordinator that 1.30am is estimated time of arrival in London and 2am at hospital.

22.45-23.00McGiverns arrive at Sligo. Coast Guard tells HSE it cannot give arrival time in London until helicopter is airborne.

Emergency response service advises McGiverns that transfer would not go ahead because hospital needed her there by 2am.
