The Health and Safety Authority last night issued a warning after the fifth death this year at quarry sites.

The authority said quarrying and associated industries had a fatal accident rate much higher than corresponding industries such as construction.

Yesterday, Mr Tom Stanley (45), Garrengreen, Johnswell Road, Kilkenny, was killed instantly in an accident involving a large cement bulk carrier. The accident occurred at Hennessy Sand and Gravel, Sheestown, Bennettsbridge.
