London - Safety tests on the signal at the centre of the investigation into the Paddington rail disaster in London have shown that it could be partially obscured by overhead line equipment, a second interim report into the crash revealed yesterday, Rachel Donnelly reports.

The report by the Health and Safety Executive (HSE) considered that the reasons why the train passed the red signal were "likely to be complex", with early indications pointing to a series of failures "involving many contributory factors", including driver error.
