A chara, – Further to Cyril McNamee (September 28th), the difference in mentality between "car crash" and "air crash" analyses is that the airline industry views each crash as potentially preventable, whereas the car industry views some rate of accidents as inevitable. The Irish health system would be well served by the Health Information and Quality Authority (Hiqa) adopting the "air crash" approach.
