Jan Vennegoor of Hesselink lost his marker to head the Dutch league leaders in front after 27 minutes and American substitute DaMarcus Beasley scored a second for Guus Hiddink's side from close range after 69 minutes to wrap up a 3-0 aggregate win.
Monaco, beaten by Porto in last year's Champions League final, started with a flurry of pressure but were kept largely subdued after Hesselink's opener left them needing to score three times to stay in the competition, due to the away goals rule.
The Didier Deschamps coached French league side have now won just one game in the last six weeks, and that was a laboured 1-0 victory over Stade Rennes in the French Cup.
