Statement from the HSE regarding inquiry:
"The HSE is establishing an investigation team to comprehensively review service provision by the HSE to Ms Evelyn Joel deceased. The investigation team is external to the southeast area. Ms Joel died in Wexford General Hospital on January 7th, 2006. The HSE extends its deepest sympathy to the family and friends of the deceased patient.
Terms of reference are currently being finalised for the investigation. The team members comprise a medical consultant, former deputy health board CEO, and healthcare professional with an extensive background in public health nursing and community service.
Terms and reference for the inquiry and members of the investigation team are currently being finalised, and will be issued once this process is completed in the next 24 hours."
