Accountability in the health service

Sir, – The tone of Minister for Health Leo Varadkar's disappointing email on the trolley crisis exposes once again the difficulty we have with accountability in public service ("Varadkar warns of sackings if trolley crisis continues", Front Page, September 29th). "Heads rolling" is not accountability. At one extreme, it is revenge; at the other, it is an executive response to a failure to give an adequate account for the discharge of responsibilities entrusted. Only when our public governance is ready to explore the subtleties of that dynamic will we begin to reverse such managerial and organisational failures as increasing numbers of patients waiting on trolleys.– Yours, etc,
