HSE investigates patient deaths

The Health Service Executive (HSE) is reviewing the deaths of two patients at psychiatric units in Dublin and Tralee.

The Health Service Executive (HSE) is reviewing the deaths of two patients at psychiatric units in Dublin and Tralee.

One patient died at the executive's acute mental health unit on the grounds of Tallaght Hospital in south Dublin, the HSE said in a statement tonight.

"As in all such incidents a review will be carried out in accordance with HSE policy," it said.

The executive "extends its sincere sympathy to the patient's family and will continue to offer support to them" the statement added.

The acute psychiatric unit in Tallaght has three wards and a total of 52 beds.

A second review is being carried out into the death of a patient at the mental health unit in Kerry General Hospital in Tralee.

The HSE South said it is in contact with both the patient's family and the gardaí.
